A patient, who presented with acute respiratory distress with initial diagnosis of interstitial lung disease, was later diagnosed to have AIDS on the basis of radiological suspicion of Pneumocystis carinil pneumonia. A high index of suspicion in patients with bilateral pneumonic shadows is emphasized.
